BSN or higher and an active RN license are required, with 1+ year of case management experience preferred. The position offers day shifts with full-time, part-time, or per-diem options, and a benefits package including healthcare, 401K, PTO, and tuition assistance.

Qualifications

• Bachelor of Science in Nursing (BSN) or higher from an accredited program.
• Active and unencumbered Registered Nurse license.
• 1+ years experience in case management, preferably in healthcare or hospital setting.
• Exceptional interpersonal and communication skills to collaborate with medical professionals, patients, and families.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive assessments of patients including medical history, medications, treatment plans, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement care plans.
• Monitor and evaluate patient progress, adjust plans as needed and communicate changes to the team.
• Serve as main point of contact between patient and healthcare providers for coordination of care.
• Maintain up-to-date records and case notes while ensuring confidentiality and compliance.
