Qualifications

• BSN or higher from an accredited program.
• Active and unencumbered RN license.
• 1+ years experience in case management, preferably in a healthcare or hospital setting.
• Excellent interpersonal and written/verbal communication to collaborate with medical teams, patients, and families.
• Proficiency in electronic health records (EHR) software.

Responsibilities

• Conduct comprehensive patient assessments, including medical history, medications, treatment plans, and psychosocial needs.
• Collaborate with healthcare providers, patients, and families to develop and implement individualized care plans.
• Monitor and evaluate patient progress, adjusting care plans as needed and communicating changes.
• Serve as main point of contact between patient and providers to ensure seamless communication and care coordination.
• Maintain up-to-date records and case notes in compliance with confidentiality standards.
