Daily Responsibilities

• This case manager will be working in office not in the field.
• The RN Case Manager is responsible for promoting a collaborative practice among the health care team by coordinating the integrated utilization management and discharge planning program including monitoring, evaluating and implementing steps to improvement.

Hours for this Position

Varied shifts, all 5 days a week, 8 hours
