Job summary

Maine Medical Center is seeking a Case Manager I to support an interdisciplinary team in facilitating patient care, improving clinical outcomes and patient satisfaction while managing care costs. The role integrates utilization management, care facilitation and discharge planning under supervision in MMC policies.

Responsibilities include coordinating care across the continuum, coaching documentation, and communicating with payors and the team to promote efficient resource use and quality care.

Qualifications

• BSN or matriculate into BSN within one year and complete within five years.
• Current Maine RN license; professional Case Manager certification preferred.
• 1–3 years clinical practice; 3–5 years preferred.
• Excellent interpersonal communication and negotiation abilities.
• Strong analytical, data management and PC skills.
• Knowledge of discharge planning, utilization, case management and managed care reimbursement.
• Understanding pre-acute and post-acute care venues and community resources.

Responsibilities

• Facilitate collaboration across the continuum of patient care.
• Address barriers to timely and efficient care delivery and reimbursement.
• Support and coach clinical documentation efforts.
• Coordinate communication with the interdisciplinary care team.
