Job summary

Eligible candidates should hold RN licensure or equivalent, with CM preferred and 3–5 years of clinical experience. Strong communication and data-management skills are essential for success in this high-demand setting.

Qualifications

• RN or other applicable clinical licensure required; bachelor's degree and/or CM certification preferred.
• Recent sound clinical experience of 3–5 years with a target population.
• Demonstrated data management, problem solving and communication abilities.

Responsibilities

• Assesses, plans, implements, coordinates, monitors and evaluates options and services to meet health needs.
