Job summary

Carilion Clinic in Roanoke, VA is seeking an experienced RN Case Manager for the ED Observation Unit. This full-time weekends-only role focuses on coordinating patient care, discharge planning, and resource management to optimize patient flow.

You will assess patient needs, collaborate with the interdisciplinary team, document thoroughly, and facilitate transitions to post-acute services while complying with regulatory guidelines.

Qualifications

• RN license and state requirements for Virginia.
• Bachelor's degree in Nursing required.
• Three years of recent clinical health care experience preferred.

Responsibilities

• Perform patient needs assessments at admission and during care.
• Coordinate referrals and linkages to post-acute services.
• Promote interdisciplinary collaboration to progress the care plan.
• Ensure timely discharge planning and proper documentation.
• Communicate with UR nurses and social work partners daily.
• Document in medical records to reflect plan of care and status.
