Job summary

Carilion Clinic in Roanoke seeks a full-time RN Case Manager to support diverse patient populations across the care continuum. The role runs Monday–Friday, 8am–4:30pm, with a sign-on bonus and relocation assistance to help you join our team.

The patient-care focus includes assessments, referrals to home health, hospice, and DME, and active collaboration with an interdisciplinary team to optimize outcomes and reduce fragmentation of care.

Qualifications

• Registered Nurse licensed in Virginia.
• Bachelor's degree required; 5 years RN hospital experience may substitute for a bachelor’s degree.
• Current BLS-HCP (AHA) certification within 6 months of hire.

Responsibilities

• Coordinate care and services within the case-managed population.
• Perform face-to-face patient assessments and document plans of care.
• Collaborate with Utilization Review Nurse and Social Work partners.
• Ensure timely discharges and transitions across the care continuum.
