Job summary

Carilion Clinic is seeking a registered nurse to perform weekend RN case management. You will assess patients, coordinate care, and collaborate with Utilization Review, social work, and other disciplines to optimize outcomes and reduce length of stay.

Responsibilities include ensuring accurate documentation, facilitating post-acute referrals, and participating in interdisciplinary rounds and performance improvement initiatives.

Qualifications

• Registered Nurse with Virginia license.
• Bachelor's degree required; 5 years RN hospital experience may substitute.
• Experience in case management, discharge planning, collaboration with UR/SW teams.

Responsibilities

• Provide patient population case management and care coordination.
• Collaborate with Utilization Review Nurse and Social Work.
• Use InterQual to support patient status and medical necessity.
• Coordinate post-acute referrals (home health, hospice, DME).
• Facilitate discharge planning and patient/family communication.
• Participate in interdisciplinary team meetings and performance improvements.
