Job summary

Qualifications

Responsibilities

• Provides case management for assigned patient populations.
• Promotes interdisciplinary collaboration to progress care and discharge plan.
• Performs patient needs assessments upon admission and at regular intervals.
• Coordinates post-acute referrals (home health, hospice, DME).
• Maintains documentation reflecting patient condition and plan of care.
• Communicates with patients/families about financial implications of discharge plans.
