Qualifications

Responsibilities

• Research and design treatment and care plans to promote quality and cost-effective services.
• Initiate timely individualized care plans based on HRA and participate in interdisciplinary meetings.
• Determine referrals for CM services, mental health, and social services.
• Provide referrals to QM, DM, and AGD departments.
• Review member care with medical groups or providers for continuity of care.
