Job summary

One Brooklyn Health System Inc. is seeking a Case Manager to assess medical necessity for admissions and ongoing stays, coordinating care with multidisciplinary teams.

Responsibilities include EHR-based reviews, discharge planning, and timely communication with physicians, social workers, and insurers. Requirements include a BSN, NY RN license, BLS, PRI certification, and at least 3 years of clinical experience in acute or similar settings.

Qualifications

• Knowledge of Federal, State and JCAHO guidelines for utilization review, discharge planning/quality assurance/infection control.
• Minimum three (3) years clinical experience in one or more of the following: Acute care, Medical-Surgical, Intensive Care, Emergency Department, Community Nursing (Home Care).
• Current New York State Registered Nurse licensure and Basic Life Support (BLS) certification.
• PRI certification required; PALS and CCM certifications preferred; ACLS preferred.

Responsibilities

• Performs face-to-face visits to assess physical, mental, and emotional barriers to self-care and initiates referrals to social work.
• Daily EHR review of patient medical necessity (severity of illness and intensity of service) and participation in bedside or interdisciplinary rounds.
• Collaborates with social workers on social determinants of health that could affect outcomes.
• Communicates with Attending Physicians on admission guidelines and ensures timely, compliant care planning.
