Qualifications

• Graduate of an accredited school of nursing.
• Current license to practice as a Registered Nurse in the State of Nevada.
• Knowledge of social and physical factors affecting discharge and community resources.

Responsibilities

• Achieve quality healthcare outcomes by establishing safe discharge planning.
• Provide timely utilization management services to maximize reimbursement for patient visits.
• Collaborate with patient, family and healthcare team to ensure appropriate post-discharge care.
