The ideal candidate holds an active Nevada RN license, at least three years of varied clinical experience, and familiarity with discharge planning, utilization review

Qualifications

• RN license in Nevada is required.
• Minimum three years of varied clinical experience; UR/UM/Case Management 2+ years preferred.
• Knowledge of social and physical factors affecting discharge and community resources.
• Computer proficiency for data collection/management.

Responsibilities

• Establish safe, individualized discharge plans and proficient timely utilization management to maximize reimbursement.
• Collaborate with patient, family, and healthcare team for post-discharge care and outcomes.
