Job summary

Valley Health System in Southern Nevada seeks an experienced registered nurse to lead discharge planning and provide timely utilization management to maximize reimbursement and ensure quality care. You will collaborate with patients, families and the healthcare team to support safe transitions.

Requirements include graduation from an accredited nursing program, Nevada RN license, and at least three years of clinical experience with two years in utilization review or case management preferred;

Qualifications

• Graduate of an accredited school of nursing.
• Three years of varied clinical experience; two years in Utilization Review/Utilization Management or Case Management preferred.
• Knowledge of social and physical factors affecting discharge and community resources to meet post-discharge needs.
• Proficiency with word processing, spreadsheets and data management software.

Responsibilities

• Establish safe, individualized discharge plans and provide proficient timely utilization management to maximize reimbursement and care quality.
• Collaborate with patient, family and healthcare team to achieve optimal health outcomes.
• Ensure age-appropriate care planning and documentation across the patient care continuum.
