Job summary

SnapCare is seeking a Case Manager in Rochelle Park, NJ for a 13-week assignment with day shift hours from 8:00 AM to 4:30 PM. The role involves identifying patient and family needs, coordinating resources, and guiding a clinical plan of care with an interdisciplinary team to ensure timely discharge.

Applicants should have at least 2 years in case management and discharge planning in acute care, with current licensure in a clinical discipline (RN/RT/SW preferred) and knowledge of insurance

Qualifications

• 2+ years of Case Management and Discharge Planning experience in an acute care setting.
• Current licensure in a clinical discipline per state guidelines (RN, RT, or SW preferred).
• Knowledge of the insurance industry and government reimbursement.

Responsibilities

• Identify patient problems or situations affecting the patient and/or family’s ability to adapt to or participate in their treatment plan.
• Assist patients and families in utilizing hospital resources to remove barriers and adjust to their healthcare situation.
• Collaborate with interdisciplinary team, including physicians, to manage and direct a clinical plan of care to achieve safe and timely discharge.
• Conduct utilization review and ensure medical necessity review criteria for all admissions and continued stays.
• Manage discharge planning evaluations per policy and regulatory standards.
• Interact appropriately with patients according to age and level of understanding.
• Refer patients and families to outside agencies for social problems as required by state regulation.
• Maintain up-to-date community resource system and assist patients and families in accessing appropriate services.
• Ensure timely insurance certifications and payor requirements are met.
• Document plan of care and progress notes in the medical record per policy and state regulations.
