Job summary

Christiana Care Health System in Newark, DE is seeking an experienced RN Case Manager for day shift. The role focuses on discharge planning within an acute care setting, coordinating care with an interdisciplinary team to ensure timely transitions.

External candidates may receive a 10K sign-on bonus and the schedule is Mon-Fri with no holidays or weekends. Requirements include an active RN license in DE or a compact state, BSN, and 3–5 years of acute care experience; a CMC must be obtained

Qualifications

• Active RN license in DE or a Compact State.
• BSN is required.
• 3-5 years of acute care hospital experience.
• In-patient case management experience preferred.
• 3-5 years of acute care experience required.
• BLS certification preferred.
• Case Management Certification (CMC) must be obtained within 18 months of eligibility.

Responsibilities

• Coordinate care and drive patient progression to establish a discharge plan.
• Function as an interdisciplinary team member to develop discharge planning.
• Identify system barriers and help implement strategies to remove them.
• Collaborate with physicians, nurses, social workers and payers for transitions.
