Qualifications

• 2 years of experience in Case Management and Discharge Planning in an Acute Care setting.

Responsibilities

• Identify problems affecting the patient and family's ability to adapt to their healthcare situation.
• Assist patients and families in utilizing hospital resources to remove barriers to treatment.
• Facilitate patient and family participation in the decision-making process regarding the plan of care.
• Manage and direct clinical plans of care in collaboration with the interdisciplinary team.
• Ensure timely completion of documentation and compliance with payor guidelines.
• Maintain a community resource system to assist patients and families in accessing services.
• Monitor and verify medical necessity for admissions and continued stays.
• Handle referrals to outside agencies for social issues as needed, following state regulations.
• Participate in team conferences and educational meetings to improve operations.
