Inova Fair Oaks Hospital is seeking a dedicated Registered Nurse Case Manager for a Full Time Day shift, Monday–Friday with rotating weekends and holidays. The role focuses on discharge planning and coordinating care across acute and post-acute settings, serving as the primary liaison among patients, families and the healthcare team.

The successful candidate will have a Virginia RN license and at least 1 year of case management or clinical care experience, with BSN preferred or a plan to

Qualifications

• Certification: Basic Life Support (AHA).
• Licensure: Licensed or eligible for licensure in Virginia as a Registered Nurse.
• Experience: 1 year of case management and/or clinical care experience.
• Education: BSN required within 5 years if ADN holder.

Responsibilities

• Collects data for performance and outcome indicators.
• Collaborates with disciplines to ensure effective patient management.
• Performs discharge planning and coordinates post-discharge services.
• Acts as liaison between patients, families and healthcare team.
• Educates patients and families and addresses barriers to care.
• Discusses payer criteria with clinical staff and follows up with payers.
