Job summary

Inova Fairfax Hospital in Falls Church, VA, is seeking an Experienced Registered Nurse Case Manager 1 to join the Case Management Team. This full-time role involves discharge planning, care coordination, and serving as a liaison between patients, families, and the healthcare team.

You will apply utilization management to evaluate medical necessity and facilitate precertification and payer authorization when needed.

Qualifications

• BSN required or ADN with plan to complete BSN within 5 years.
• Basic Life Support certification.
• Licensed or eligible for Virginia RN licensure.
• Minimum 1 year of case management or clinical care experience.

Responsibilities

• Provide discharge planning and continuity of care for acute and post-acute patients.
• Coordinate referrals to clinics, home health, hospice and rehab.
• Collaborate with interdisciplinary teams to plan care and transition.
• Apply utilization management to determine medical necessity and efficiency.
• Ensure regulatory compliance and precertification when indicated.
• Educate patients and families and address barriers to care.
