Job summary

SSM Health St. Mary’s Hospital - Madison is seeking a Registered Nurse to assess, develop, plan, implement, and evaluate discharge planning needs in collaboration with the interdisciplinary team.

The role emphasizes ensuring CoP discharge planning compliance, identifying barriers to length of stay, and coordinating with post-acute providers to transition patients to the next level of care. Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing
• Three years’ acute care experience

Responsibilities

• Ensures compliance with CoP discharge planning.
• Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
• Assesses high-risk patients for readmission and develops discharge plans with multidisciplinary team.
• Participates with team regarding opportunities for improvement in standard work.
• May be responsible for utilization management, ED case management, admission processes, and liaison with post-acute providers.
• Applies evidence-based practice in nursing care per department Scope of Service.
• Demonstrates professional nursing standards per ANA Code of Ethics.
