Qualifications

Responsibilities

• Advocate for patients’ needs and rights within health and social services.
• Conduct comprehensive assessments to determine patient needs and goals.
• Coordinate referrals to healthcare providers and community resources.
• Deliver and explain termination of benefits letters to patients.
• Ensure understanding by family when the patient cannot comprehend.
• Develop individualized care plans with patients and professionals.
• Maintain accurate records of interactions, assessments and plans.
• Inform patients and families about services and strategies to manage conditions.
• Monitor progress and adjust discharge plans as needed.
• Work with multidisciplinary teams to ensure comprehensive care.
