Job summary

SSM Health Saint Louis University Hospital is seeking a Regular part-time RN to assess discharge planning needs and collaborate with an interdisciplinary team. You will identify high-risk patients, develop care transitions, and ensure adherence to discharge planning CoP across inpatient and observation settings.

The role emphasizes care coordination with post-acute providers, utilization and admission processes, and adherence to the professional practice model.

Qualifications

• Graduate of accredited school of nursing.
• Three years' acute care experience.
• RN licensure in IL, MO, OK, or WI as required by location.

Responsibilities

• Assess discharge planning needs and collaborate with the interdisciplinary team.
• Develop discharge plans and coordinate with post-acute providers.
• Identify gaps/barriers to length of stay and readmission risks.
• Ensure compliance with relevant CoP discharge planning requirements.
