Job summary

Qualifications

• State licensure as a Registered Nurse (RN).
• Bachelor's degree in nursing from accredited educational institution, or actively pursuing degree and to be obtained within five years of accepting position.
• Three years of acute hospital care experience.

Responsibilities

• Coordinate care assessments within 24 hours of admission.
• Collaborate with social workers and other disciplines to ensure timely transition to appropriate level of care.
• Assess patient and family needs to reduce barriers and plan discharge.
