Job summary

Qualifications

• RN licensure is required.
• Bachelor’s degree in nursing (or actively pursuing with completion within five years).
• Three years of acute hospital care experience.

Responsibilities

• Performs care coordination assessments for initial assessment and transitions.
• Collaborates with social workers and other disciplines for safe, timely transitions to next level of care.
• Assesses patient/family needs to overcome barriers and formulate discharge plans.
• Identifies unsigned level of care orders and communicates with utilization management.
• Reviews DRG/LOS to plan discharge with providers and identifies the point of contact.
• Assesses readmission risk and initiates interventions to improve transitions.
• Coordinates discharge planning referrals to social services, outpatient case management, DME, post-acute placement, and other agencies.
