Abington Memorial Hospital seeks a Case Manager RN to coordinate care for a defined patient population and drive discharge planning.

You will collaborate with an interdisciplinary team, advocate for patients and families, and monitor progress to ensure timely, safe discharges.

Requirements: Bachelor of Nursing, PA RN license, and at least 3 years in case management or discharge planning; strong communication and conflict-resolution skills.

Qualifications

• Bachelor's Degree of Nursing required.
• 3 years case management or discharge planning experience required.
• RN license in Pennsylvania required.
• Strong communication and conflict resolution skills required.
• Interdisciplinary team approach to care planning and discharge coordination required.

Responsabilités

• Coordinates all aspects of discharge planning and establishes safe discharge plans.
• Advocates for patient and family throughout the episode of care.
• Facilitates communication among the healthcare team and patient/family.
• Guides patient progression during the hospital stay and supports movement through care transitions.
• Reviews length of stay and barriers to facilitate progression during multidisciplinary rounds and escalation meetings.
• Identifies barriers to care and escalates unresolved issues to Department Leadership.
