Job summary

Qualifications

• RN license in Oklahoma or multi-state license.

Responsibilities

• Plans patient care with physicians and the healthcare team.
• Ensures care and discharge plans align with clinical course and needs.
• Identifies barriers to discharge and works to resolve them.
• Participates in multidisciplinary patient rounds.
• Coordinates discharge planning with team, patients, and caregivers.
• Obtains payer authorization for post-acute services.
• Arranges care settings (home health, rehab, SNF).
• Coordinates hospital-to-hospital transfers as needed.
• Monitors readmission reasons and reports trends to leadership.
