Job summary

IHA is seeking a Registered Nurse Case Manager in a part-time, day-shift role to lead discharge planning and care coordination across the acute care episode. You will work to achieve clinical, financial and utilization outcomes while collaborating with patients, families and an interdisciplinary team.

Required: Michigan RN license, BSN, and 3 years of direct patient care with case management. CMSA certification is desirable and may be pursued within three years of employment.

Qualifications

• BSN is required.
• Current Michigan Nursing License.
• Three (3) years of direct patient care experience as an RN with case manager experience.
• CMSA certification is desirable.
• CMSA certification within three years of employment or in process.

Responsibilities

• Conduct focused assessments of patient goals and discharge needs.
• Review appropriateness, intensity, and timeliness of level of care and documentation.
• Develop and document discharge plans in collaboration with patient, family and inter-disciplinary team.
• Discuss plan of care daily with physicians and care team to determine post-acute setting.
