Job summary

Advocate Health is seeking a part-time Case Manager (Nursing) to support discharge planning and care coordination. The role requires an active RN license, BSN, and at least 5 years of clinical experience with a preference for Case Management background.

You will assess patient needs, develop care plans, and collaborate with a multidisciplinary team to optimize outcomes while managing costs. The position involves first-shift hours with benefit-eligible options.

Qualifications

• Graduate from an accredited school of nursing.
• BSN required.
• MSN preferred.
• Minimum 5 years of relevant clinical experience.
• Previous Case Management experience preferred.
• Current licensure to practice as a Registered Nurse in the applicable state.

Responsibilities

• Identifies patients who would benefit from Case Management based on discharge needs assessment.
• Assesses data and interviews patients/families to evaluate needs.
• Develops and implements a Plan of Care with nursing to improve client outcomes and align with goals.
• Collaborates with patients, caregivers, and multidisciplinary team to execute the Plan of Care.
• Coordinates discharge planning and monitors effectiveness through plan adjustments as needed.
