Responsibilities

• Lead multidisciplinary planning and coordination of patient care across the healthcare continuum.
• Organize the sequence of patient care for maximum effectiveness and efficiency to reach quality, clinical, and cost outcomes.
• Coordinate with physician, social work, providers, community resources, and payers to manage cases.
• Provide discharge planning coordination and intervention with the clinical team and medical providers.
• Promote patient wellness and improve care outcomes while minimizing denials of payment.
• Communicate and update leadership on cases requiring executive approval including data collection and reporting.
• Demonstrate Service Excellence in all duties.
