Job summary

Qualifications

• Minimum of three (3) years of case management experience.
• At least two (2) years of leadership experience within case management.
• Graduate of an accredited RN program or BSN.
• Certification in Case Management preferred.

Responsibilities

• Oversee day-to-day case management and discharge planning operations ensuring quality and efficiency in line with regulatory standards.
• Coordinate budgets and audits for the department.
• Collaborate with multidisciplinary teams to optimize patient flow and outcomes.
• Participate in clinical performance improvement activities and develop clinical paths and staff education plans.
