Responsibilities

• Provide leadership in multidisciplinary care planning and coordination across the healthcare continuum.
• Organize patient care sequence to optimize quality, clinical outcomes and cost efficiency.
• Coordinate case management with physicians, social work, and payers to minimize overlap and delays.
• Conduct utilization management and discharge planning to promote wellness and appropriate use of services.
• Report case updates and outcomes to leadership; perform other duties as assigned.
