Job summary

SSM Health St. Mary’s Hospital – Madison is seeking an experienced nurse responsible for discharge planning across the care continuum.

You will assess, plan, implement, and evaluate patient discharge needs in collaboration with the interdisciplinary team to ensure safe transitions at the next level of care. Requires a valid RN license in Illinois/Missouri/Oklahoma/Wisconsin with compact state eligibility and at least three years of acute care experience; responsibilities include utilization

Qualifications

• Graduate of accredited school of nursing or education equivalency for licensing.
• Three years’ acute care experience.
• RN licensure in multiple states with compact eligibility (IL, MO, OK, WI).

Responsibilities

• Ensures compliance with discharge planning CoP.
• Identifies gaps/barriers to inpatient/observation length of stay.
• Assesses high-risk patients for readmission; develops plan with multidisciplinary team.
• Collaborates with team on opportunities for improvement in standard work.
• May handle utilization management, ED case management, admission processes, and post-acute liaison duties.
• Applies evidence-based practice to nursing care and age-specific patient needs.
• Adheres to professional nursing standards and ANA Code of Ethics.
• Maintains alertness and safety in daily duties.
• Performs other duties as assigned.
