The Valley Health System is seeking a Registered Nurse to lead discharge planning and utilization management to ensure safe, efficient patient transitions and optimal reimbursement. You will work with patients, families and the health care team to coordinate care and resources across units.

Three years of varied clinical experience are required, with two years in utilization review/management or case management preferred.

Qualifications

• Graduate of an accredited school of nursing.
• A minimum three years experience in varied clinical settings. Two years experience in Utilization Review, Utilization Management or Case Management preferred.

Responsabilités

• Discharge planning to ensure safe, individualized patient care at discharge.
• Provide proficient timely utilization management services to maximize reimbursement for patient visits.
• Collaborate with patient, family and healthcare team to achieve quality healthcare outcomes.
