Job summary

St. Charles Health System is seeking an RN Case Manager to provide clinically based case management, discharge planning, and utilization management across multiple Bend/Prineville/Redmond/Madras locations in Oregon.

The role emphasizes collaboration with the care team, patient education, and quality improvement to improve outcomes. The position requires an Oregon RN license, BLS, and a BSN in preferred; minimum 3 years of acute care experience is expected.

Qualifications

• Graduate of an accredited school of nursing is required.
• BSN is preferred.
• 3 years of clinical nursing experience in an acute care setting is expected.
• Knowledge of Quality Improvement is beneficial.
• Experience with disease management and education for patients and staff is preferred.

Responsibilities

• Manage discharge planning and utilization management for assigned patients.
• Collaborate with physicians, nurses, and care team to coordinate care and resources.
• Document patient care accurately per SCHC policies and procedures.
• Identify barriers to discharge and facilitate resolution with the care team.
• Provide patient and family education to support post-discharge care.
