Job summary

Qualifications

• A minimum of two (2) years’ experience as an RN, or two (2) years’ experience as a Case Manager (RN or LVN).
• Strong clinical judgment and problem-solving abilities.
• Excellent written and verbal communication with patients, families and staff.

Responsibilities

• Reviews clinical information daily to determine discharge readiness.
• Coordinate discharge planning with physicians and the care team.
• Attend rounds to remove barriers to discharge and manage length of stay.
• Communicate patient status and care level in huddles and rounds.
• Collaborate with social work to assess discharge needs and plans.
