Job summary

Memorial Hospital of Gardena is seeking a Nurse Case Manager to oversee clinical resource utilization and documentation affecting reimbursement. You will monitor patient needs across medical, financial, psychosocial, and discharge planning domains, contributing to department operations and patient communication using AIDET principles.

Minimum two years RN or Case Manager experience required; BSN preferred. Licensure: RN and BLS.

Qualifications

• A minimum of two (2) years’ experience as a RN, or two (2) years’ experience as a Case Manager (RN or LVN), required.
• Critical thinking, service excellence and good interpersonal communication skills, ability to read/comprehend written instructions, strong organizational skills, ability to follow verbal instructions, and PC (computer) skills.
• Capacity to learn, synthesize, make critical judgments, work independently, place patients and families first, and collaborate with the team members who are recognized leaders within health care.

Responsibilities

• Review clinical information daily to determine continued stay and discuss discharge plan with physician and team.
• Direct discharge planning and coordination with related staff to arrange post-acute facility, DME, home health, etc., timely.
• Attend HPIRounds to remove barriers to discharge and manage LOS.
• Communicate patient status, level of care, and LOS in huddles and rounds.
• Initiate discharge planning with social work within 24 hours of admission and collaborate with care team to create safe transitions.
