Qualifikationen

• Associate’s Degree in Nursing from an accredited program.
• Minimum 5 years of clinical acute care nursing experience.
• At least 2 years of case management experience within the past five years.
• Knowledge of case management principles and healthcare management.
• Strong problem-solving and multitasking abilities.

Aufgaben

• Manage a caseload of 18-20 patients.
• Assess physical and biopsychosocial needs through clinical assessment and data.
• Develop a plan of care in collaboration with patient, family, and care team.
• Initiate discharge planning assessment within 24 business hours of admission.
• Participate in interdisciplinary meetings and rounds.
• Complete Medicare One Day Stay and disposition forms timely.
• Document payer communication and authorization in the records.
• Perform InterQual assessments on admission, changes in care level, at least every three days, and discharge.
