Job summary

Community Hospital of Huntington Park in Huntington Park, CA is seeking a Nurse Case Manager to oversee clinical resource utilization and documentation that affects reimbursement. Under the CM Director, you will evaluate patient needs and plan discharge with a focus on timely transitions.

The role requires at least two years as RN or Case Manager, collaboration with social work, discharge planning, and adherence to AIDET and HIPAA.

Qualifications

• A minimum of two (2) years’ experience as a RN, or two (2) years’ experience as a Case Manager (RN or LVN), required.
• Critical thinking, service excellence and good interpersonal communication skills.
• Ability to read/comprehend written instructions, strong organizational skills, and PC (computer) skills.

Responsibilities

• Reviews clinical information daily to determine if continued stay is clinically appropriate and discuss the discharge plan with the physician and team.
• Partner with the Discharge Planning Assistant to direct the facilitation of transition/discharge plan to arrange post-acute facility, DME, HH, etc. in a timely manner, preventing avoidable days.
• Attend daily HPIRounds to collaborate with health care team in removing barriers to discharge and managing LOS.
• Communicate current patient status, level of care and LOS in huddles and HPIRounds.
• In conjunction with Social Work, interview patients/family within 24 hours of admission to determine discharge needs and initiate the discharge plan.
• Collaborate with other care team members to create a safe and effective transition/discharge plan.
• Respond to case management consults for patients.
• Refer patients to SW in a consult as indicated.
• Confers with physician for clarification/change of status order when indicated; ensure status order written in EHR.
• Collaborates with CDI in determining GMLOS to drive length of stay.
• Initiates referral to financial counseling for patients needing assistance.
• Completes Avoidable Days.
• Consults with medical staff, nursing, and ancillary staff regarding patient needs.
• Uses clear, concise, professional communication and applies AIDET principles.
• Collaborates across disciplines to coordinate patient care, including transfers, discharge, referrals, and psychosocial support.
• Acts with urgency when performing tasks.
• Maintains organization and cleanliness of files and work area.
• Reports equipment/environment issues for repair.
• Adheres to HIPAA and uses SBAR for physician communications.
• Speaks up to stop the line and escalate safety events.
