Alta Hospital in California is seeking an RN Case Manager to perform clinical assessments for utilization review, resource management and safe discharge planning. You will collaborate with an interdisciplinary team to promote cost-effective patient care and smooth transitions of care, while advocating for patient self-determination and choice.

The role requires RN licensure in California, experience in case management or nursing leadership, and strong knowledge of care coordination, CMS

Formación

• One year of case management or nursing leadership experience.
• Current Licensure as a Registered Nurse in California.
• In-depth knowledge of patient throughput, care coordination, and care planning processes.
• Ability to assess medical stability for discharge and evaluate medical necessity for continued hospitalization.
• Computer/EMR proficiency and literacy.
• Knowledge of CMS, Medicare, Medi-Cal and Managed Care reimbursement.
• Familiarity with Joint Commission, CMS, CDPH requirements.
• Excellent written and verbal communication skills in English.

Responsabilidades

• Collaborates with patients and families to optimize outcomes and coordinates transfers, discharges and transitions of care.
• Reviews medical necessity using evidence-based criteria to determine level of care and length of stay.
• Documents and communicates care coordination findings to the payer and supervisor in a timely manner.
• Leads and participates in interdisciplinary rounds and discharge planning assessments.
• Acts as liaison to medical staff to ensure continuity of services according to the plan of care.
• Participates in patient/family meetings and documents informed decision making.
