Job summary

Kaiser Permanente in Los Angeles, CA seeks a Registered Nurse to collaborate with an MD to coordinate admissions, screen medical necessity for inpatient care, and develop safe discharge plans with families.

You will act as patient advocate, coordinate with multidisciplinary teams, and ensure compliance with TJC, CMS, and state regulations while maintaining high quality and cost-effective care. Requires CA RN license and 2 years acute care or case management experience.

Qualifications

• Two (2) years RN experience in acute care or case management.
• Completion of an accredited RN training program that allows graduates to take RN license exam.
• Registered Nurse License (California).
• Basic Life Support certification.
• Demonstrated ability in utilization review, discharge planning or case management.
• Working knowledge of regulatory requirements (TJC, Medicare, Medi-Cal, etc.).
• Strong written and verbal communication, interpersonal, critical thinking and problem-solving skills.
• Planning, organizing, conflict resolution and negotiating skills.
• Computer literacy skills.

Responsibilities

• Plans, coordinates, and assesses care provided to members.
• Collaborates with physicians and the multidisciplinary team to document appropriate care plans.
• Recommends alternative levels of care and ensures compliance with requirements.
• Develops discharge plans and communicates them to physicians, patient, and family.
• Coordinates transfer of patients to appropriate facilities and ensures required authorizations.
