Inova Loudoun Hospital is seeking a dedicated Registered Nurse Case Manager 1 to join the Case Management Team as a full-time role, Monday through Friday with rotating weekends. The RN Case Manager will provide discharge planning and continuity of care for patients in acute and post-acute settings, coordinating services and liaising with patients, families, and the care team.
