Qualifications

Responsibilities

• Provide discharge planning and care coordination for assigned patients.
• Act as liaison between patients, families and the healthcare team.
• Apply utilization management to determine medical necessity and efficiency.
• Support payer authorization processes and documentation in records.
• Participate in performance improvement activities.
