Job summary

Northeastern Nevada Regional Hospital in Elko, NV is seeking an RN Case Manager (Part-time, varied shifts) to join our Case Management team. The role focuses on coordinating discharge planning, utilization review, and patient advocacy to promote safe, effective care during hospital stays.

The RN license and a nursing degree are required, with BSN preferred; BLS and ACLS are mandatory, and 1 year of experience is preferred. New grads welcome to apply.

Qualifications

• Current Nevada RN license (Compact License N/A in Nevada) and an accredited nursing school degree; BSN preferred.
• Basic Life Support and ACLS certification required.
• One year of experience preferred.

Responsibilities

• Consult with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care.
• Coordinate discharge planning and patient transfers to other facilities.
• Document case management activities, plans and outcomes.
• Monitor patient and family satisfaction and respond to questions/concerns.
• Lead utilization review activities and ensure timely care progression.
• Participate in performance improvement and continuing education.
