Job summary

Nevada Health Centers, Inc. is seeking an experienced RN Case Manager to provide patient-centered case management in a Pahrump, NV Health Home setting. You will coordinate care, educate patients, and collaborate with providers to improve outcomes and reduce costs.

Requires NV RN licensure, CPR/BLS, 3+ years clinical experience (ER/ICU preferred), and proficiency with EHR systems. This is a full-time, on-site position with opportunities for professional growth.

Qualifications

• Graduate of an accredited nursing school; BSN preferred.
• Active Nevada RN license required.
• Current CPR/BLS certification required.
• At least three years of clinical practice experience (ER/ICU).
• Familiarity with Electronic Health Record systems.

Responsibilities

• Manage care coordination for a defined patient population.
• Educate patients and coach on treatment options.
• Assist in selecting physicians and facilities to optimize access and cost.
• Coordinate referrals to health programs and manage inbound/outbound calls.
• Track patients receiving services outside the practice and suggest alternatives.
• Follow up after ER visits or hospital discharges for high-risk patients.
• Identify high-risk patients for improved outcomes.
• Manage network participation and transfers with specialists and DME providers.
• Support special projects and other duties as assigned.
• Encounter patients as needed.
