Houston Methodist is seeking a Case Manager (RN) to assess clinical condition, plan care management, and coordinate transitions on a designated unit. You will work with physicians and the care team to ensure quality, safe patient care and efficient discharge planning.

The role requires Texas RN licensure, 3 years hospital nursing experience, and strong communication skills. You will collaborate with payors and post-acute providers to ensure medically necessary admissions and timely transitions,

Competenze

• RN with Texas licensure or ability to obtain permanent Texas license within 60 days.
• Three years hospital nursing clinical experience; acute care case management preferred.
• Bachelor’s degree preferred.

Mansioni

• Assess patients timely and review medical necessity for throughput.
• Coordinate discharge planning with care team and families.
