Job summary

Qualifications

• TX RN license active
• ASN/ Nursing diploma required, BSN preferred
• 3+ years clinical hospital nursing or 2+ years case management experience
• Case management certification preferred
• InterQual experience preferred

Responsibilities

• Assess psychosocial, medical, and discharge needs of patients/families
• Develop and monitor the plan of care to promote safe discharge
• Coordinate discharge planning with multidisciplinary team and physician
• Refer to Social Services for post-acute needs as appropriate
• Advocate for patient rights, preferences, and cultural considerations
• Document care coordination activities and communication with the health care team
• Support patient throughput while ensuring quality outcomes
