Job summary

TAG MedStaffing is hiring an RN Case Manager in Staten Island, NY for our client, providing case management, discharge planning, care coordination, and utilization review within an acute care hospital environment.

The RN Case Manager will assess patient needs, coordinate transitions of care, develop discharge plans, and collaborate with multidisciplinary healthcare teams to support continuity of care and positive patient outcomes.

Qualifications

• Graduate of an accredited nursing program is required.
• Active New York State Registered Nurse license is required.
• Active PRI certification is required.
• 2+ years of RN case management experience within a hospital or acute care environment is required.
• Strong knowledge of care coordination, discharge planning, utilization management and interdisciplinary patient care.
• Experience with Meditech and CarePort is required.
• Strong clinical assessment, communication, organization, documentation, problem-solving and collaboration skills.

Responsibilities

• Coordinate patient care across the continuum using clinical guidelines and individualized care plans.
• Evaluate patient needs, barriers to care, and clinical requirements to support discharge planning.
• Collaborate with physicians, nursing teams, social services and other healthcare professionals to facilitate timely patient care.
• Coordinate post-acute services, resources, and transitions of care based on patient needs.
• Communicate with third-party representatives and internal teams to support utilization and continuity of care.
• Maintain accurate and timely case management documentation in relevant systems.

Job Overview – RN Case Manager
Compensation: $65 – $70/hour
Location: Staten Island, NY
Schedule: Monday to Friday(In-Office, Every Other Weekend Rotation)

TAG MedStaffing is hiring an RN Case Manager in Staten Island, NY for our client, providing case management, discharge planning, care coordination, and utilization review within an acute care hospital environment. The RN Case Manager will assess patient needs, coordinate transitions of care, develop discharge plans, and collaborate with multidisciplinary healthcare teams to support continuity of care and positive patient outcomes. The ideal candidate has 2+ years of hospital-based RN case management, discharge planning, utilization review, or acute care experience with strong clinical assessment, communication, and care coordination skills.

Responsibilities as the RN Case Manager:

• Case Management: Coordinate patient care across the continuum using established clinical guidelines and individualized care plans.
• Patient Assessment: Evaluate patient needs, barriers to care, and clinical requirements to support appropriate services and discharge planning.
• Care Coordination: Collaborate with physicians, nursing teams, social services, and other healthcare professionals to facilitate timely patient care.
• Discharge Planning: Coordinate post-acute services, resources, and transitions of care based on patient needs and clinical recommendations.
• Utilization Support: Communicate with third-party representatives and internal teams to support appropriate utilization, reimbursement, and continuity of care.
• Clinical Documentation: Maintain accurate and timely case management documentation within applicable clinical and care coordination systems.

Qualifications for the RN Case Manager:

• Education: Graduate of an accredited nursing program is required.
• Licensure: Active New York State Registered Nurse license is required.
• Certification: Active PRI certification is required.
• Experience: 2+ years of RN case management experience within a hospital or acute care environment is required.
• Industry Knowledge: Strong knowledge of care coordination, discharge planning, utilization management, clinical guidelines, and interdisciplinary patient care is required.
• Technical Skills: Experience with Meditech and CarePort is required.
• Skills & Attributes: Strong clinical assessment, communication, organization, documentation, problem-solving, and interdisciplinary collaboration skills are required.
