Job summary

Horizon Home Care & Hospice, Inc. in Milwaukee, WI, seeks a registered nurse for a weekday, office-based role overseeing home care and hospice patient plans. You will review OASIS, manage plans, and triage calls.

Qualifications

Graduate of an Accredited School of Nursing.
Current WI RN license
1-2 years of Med/Surg or Home Care experience
Current/Valid WI Drivers License; plus reliable/insured vehicle

Responsibilities

Reviews OASIS and plan of care documentation for accuracy.
Triages patient phone calls.
Helps with remote patient monitoring.
In office position — Milwaukee office.
Might cover PM or weekend triage for PTO/Ill calls.
