Horizon Home Care & Hospice is seeking a qualified RN for our Milwaukee office. The position focuses on reviewing OASIS and care plans, triaging patient calls, and supporting remote monitoring to ensure accurate documentation and continuous patient comfort.

The role is in-office, Monday through Friday, 8:00 am to 4:30 pm, with potential PM or weekend triage coverage for PTO or illness-related calls. Current WI RN license and BSN are required.

Qualifikationen

• Must be graduate of an Accredited School of Nursing with BSN.
• Current WI RN license required.
• 1–2 years of Med/Surg or Home Care experience preferred.

• Triages patient phone calls and helps clear issues.
• Participates in remote patient monitoring.

• Graduate of an Accredited School of Nursing. BSN required
• Current WI RN license
• 1-2 years of Med/Surg or Home Care experience
• Current/Valid WI Drivers License; plus reliable/insured vehicle
