Qualifications

• Minimum one year RN experience in acute, long-term, or home care.
• Current Wisconsin driver's license with reliable insured vehicle.

Responsibilities

• Assess patient conditions and family's conditions accurately.
• Provide patient care per physician orders and Agency policy.
• Communicate patient's issues to the appropriate healthcare professionals.
• Complete required documentation accurately and timely.
• Transcribe, implement and evaluate new orders accurately.
• Collaborate with patients, families and hospice team.
• Communicate issues to the care coordinator and clinical team.

Horizon Home Care and Hospice is seeking experienced nurses to join our compassionate and patient-centered focused Hospice Team.

As a PM Hospice Admission/ Visit Nurse you will be responsible for ensuring the delivery of quality home hospice services as well as the coordination of services that reflect the philosophy and standards of hospice nursing care of assigned patients. Provides services at quality levels consistent with professional standards of practice.

Key Responsibilities

• Assess patient conditions and family / caregiver's conditions accurately and completely during each admission or visit (conditions may include physiological, psychosocial and / or spiritual).
• Identify and provide for patient and family's educational needs.
• Provide patient care per physician orders and Agency policy.
• Complete all required patient and Agency documentation in an accurate, thorough, legible and timely fashion to reflect a clear picture of care provided.
• Transcribe, implement and evaluate new physician orders accurately.
• Work collaboratively with patients, care providers, family members and Agency staff.
• Communicate patient's issues and concerns to the Patient Care Coordinator, appropriate health care professional, hospice interdisciplinary team and / or Clinical Manager in a timely and accurate manner.

Schedule: Monday-Friday, 2nd Shift (2:00pm - 10:30pm or 12pm-8:30pm or 1pm-9:30pm), No Weekends and includes holiday rotation.
